By booking this RSVP, you acknowledge and hereby release DONAMATRIX TRAINING and its contractors or fitness trainers from any claim for damages or suit by reason of injury, illness, or damage to person or property during the course of the exercise program, including special events. Booking this workout grant full permission for DONAMATRIX TRAINING and its agents to record any or all of my participation with photos, videos, audio and/or other media known or unknown, and use them for publicity, promotions and advertising trade or commercial purposes, without reimbursement of any kind due to me now or in the future.
